https://planx.pitb.gov.pk/apply describes PlanX, the Punjab Information Technology Board accelerator, and carries the Apply Now link. The site says applications are open for a limited time without naming a closing date, so check it before starting.

Create an F6S account
Apply Now leads to f6s.com/planx-techaccelerator/apply, a third-party form host. An F6S login is needed before the application can be started, which is easy to miss because the PlanX site itself has no form.

Complete and submit the application on F6S
The whole application is filled in and submitted on F6S rather than on the PlanX site.
